LaGrange clinic gets grant for diabetes, hypertension care

Hope Health Clinic is an organization based in La Grange, Kentucky, that focuses on reducing health disparities for patients who are uninsured in the La Grange Area. 

With the help of the CVS Health Foundation and the National Association of Free and Charitable Clinics (NAFC), Hope Health Clinic recently was awarded a $20,000 grant that will allow the organization to provide primary healthcare and chronic disease management services to patients, mainly focusing on helping to manage hypertension and diabetes.

The CVS Health Foundation is a private charitable organization created by CVS Health to build healthier communities.

While partnering with NAFC, the CVS Health Foundation donated $1 million in new grants. The Hope Health clinic is one of 49 free clinics across the county that is receiving funding as part of the Foundation’s effort to make quality healthcare more convenient and affordable for Americans.

“We were honored to receive this grant, as it will allow us to provide basic medications for pa-tients dealing with hypertension and diabetes,” said Janet Warren, executive director of Hope Health Clinic. “It will also streamline our patient care services with direct medical professional visits with the hope of reducing time away from work or home and increasing the likelihood of improving their health outcome more rapidly.”

The clinic also plans on working with two schools of nursing, which will help educated nurses on the patient’s situations and will teach them how to effectively provide care through phone calls and direct follow-up visits.
