Exalta Health, formerly Health Intervention Services, has been providing quality medical services to low income patients for over 20 years. While we think what we do is important, how do we know if it is making a difference in people’s lives? As a way to measure our impact and to improve our quality, we proposed to create a system of measurable outcomes for our medical services. This webinar will present the real challenges and changes we experienced while creating a system for measuring, monitoring, and reporting outcomes. Dr. Laura VanderMolen, Exalta Health Medical Director, and Dr. Fred De Jong with Research & Evaluation Associates will present practical considerations when implementing an outcomes system in a safety-net primary care setting.
