$25 $0 - Thumb drives will generously be provided by WV Health Right, Inc.’s SCALE program (funded by the AstraZeneca HealthCare Foundation’s Connections for Cardiovascular HealthSM program)! (Anyone who already paid for a thumb drive will recieve a refund for the thumb drive cost)

Payment Information

When registering for the 2017 Charitable Health Care Symposium, you may pay by either credit card or check. You will indicate your payment preference during the online registration process.  To pay by credit card, you will enter your payment information on our secure site and the card will be processed electronically.

If you wish to pay by check, please make that selection during your registration process and follow the directions below:

  • Make checks payable to NAFC
  • Write the word "Symposium" in the memo section of your check
  • Enclose a copy of your Registration Confirmation with your check
  • Mail the document and check to:

  • Please note last day to register for the Symposium is September 22, 2017
  • Please note that all registrations must be paid in FULL prior to arriving at the Symposium – the cut off date for payment is September 25, 2017
Symposium Speaker Discount

Attendees that are selected to speak at the 2017 Symposium will receive 50% off of the registration price. 

Group Discount

When you begin your registration, you will have the opportunity to add additional group members.  For NAFC Members - when more than 3 people register in a group, the first 3 registrants will pay the full registration price and then any additional registrants added will be offered a discount of $50 off the registration price.

PLEASE NOTE: IMPORTANT - In order to receive a group discount, before registering please email Ariana Gordillo at ariana@nafcclinics.org with the total number of registrants in your group to receive a discount code and further instruction.  Also, this multi-registrant discount ONLY applies to NAFC Member Registration Pricing.   If you would like to add registrants after you have completed registration, please contact the NAFC office at info@nafcclinics.org.

Refund Policy

If canceled by September 1, 2017, the NAFC will refund 50% of the registration fee.  The NAFC will not refund any monies after September 1, 2017. Symposium Registrations that cannot be refunded cannot be used instead for NAFC membership.
